Nutrition Education Overview
How to Enroll
Choose the setting your family prefers:
One on One Appointments
What: A series of 6 appointments scheduled 1-2 weeks apart at our main program office at Physicians’ Medical Center, or our satellite office at Sunrise Family Clinic.
How: Call PMC at (503) 472-6161, SFC at (503) 474-3600, or email director@snackprogram.org and ask to schedule a SNACK enrollment appointment. You can also fill out our Referral Form and we will call you.
Group Nutrition Classes
What: A series of 3-5 weekly classes held at the McMinnville Community Center and divided by age group.
How: Check our Class Calendar or email director@snackprogram.org and ask when the next group class starts.
Cooking + Nutrition Classes
What: A series of 4 weekly cooking classes held at YCAP and divided by age group.
How: Check our Class Calendar or email director@snackprogram.org and ask when the next cooking and nutrition class starts.
FAQ
Who can participate in SNACK?
All Yamhill County residents ages 6-18 can join SNACK.
How much do appointments and classes cost?
All nutrition services are provided free of charge - we will never ask for payment information.
Can I fill enrollment forms out online?
Yes! You can fill out Enrollment Forms HERE.
What can I expect at my enrollment appointment?
During your enrollment appointment, you* will get an overview of the SNACK program and the nutrition education curriculum and incentives. We recommend all SNACK participants complete the 5-lesson Nutrition Education series, but it is not required. We will discuss your health challenges, any goals you have, where in your life you’d like support, what types of foods you prefer, and practice setting your first goal.
*Children and youth under 14 must be accompanied by a parent or guardian.
